About your story

Your story needs to be about health care.

You can share something good or something that could be better.
 

Keep it safe

Do not share anything that makes you feel upset or unsafe. If you start to feel worried, stop and talk with someone you trust.

Do not share any personal information about yourself or about other people in your story. Personal information might include your full name, address or phone number.

It is OK to share this information when you are filling out the form so we can contact you.
 

Keep it short

If you are writing your story

  • Keep it to one page

If you are sharing a video

  • keep it between 30 seconds and 1 minute
  • Make sure you take a video where it is quiet

If you are sharing your art

  • take a photo of it
  • Write the title
  • Write three to five sentences about it

  • If you could change one thing about health care, what would it be?
  • Talk about a time when you had a great experience with health care. What made it good?
  • Talk about a time when health care was hard for you. What would have helped?
  • What makes you feel respected and listened to by health care professionals?
