About this form

The Centre is forming an advisory group of family and supporters of people with intellectual disability.

You can use this form to share your interest in joining this group. 

To be a part of this group you must support a person with an intellectual disability, this includes people with intellectual disability with complex support needs.

There are some questions we will need to ask so that we can contact you.

You do not have to answer the questions after the contact details if you do not want to.

However, if you do not answer the questions, it may make it difficult for the us to decide if this group is a good fit for you.

We will use your answers to make sure we have a diverse group of people in the group.

You can ask a member of the project team for more information about how we will use your answers.
