The person you support

In this section we will be asking for sensitive information about the person you support.

This makes sure we are including the diverse voices of our community.

Consent from the person you support

Before you include information about the person you support in this form, you should support them to understand:

  • what this form is about
  • what information you would like to share about them
  • why you are sharing the information
  • how the Centre will use their information
  • who may read their information.

The person should be supported to make their own decision about whether their information is included in this form.

People communicate their decisions, wishes and preferences in different ways.

This may include:

  • speaking or writing
  • gestures or facial expressions
  • body language or behaviour
  • pictures, videos or communication devices
  • their usual ways of communicating or responding
  • support from someone they know and trust.
